Additional requirements for certain districts:

Blue Valley USD #229
→ You must sign a form giving us permission to send your transcript to Blue Valley, available in Bluemont Hall Room 13. Due by February 1, 2012.

Shawnee Mission
USD #512
→ You must sign a form giving us permission to send your transcript to Shawnee Mission, available in Bluemont Hall Room 13. Due by February 1, 2012.
→ Provide signed doctor's verification of good health (basic physical). Due March 2012.

NOTE: These forms are available and need to be returned to Bluemont Hall Room 13.
